Surgical repair of male genital lymphoedma

Summary
Objective: Genital lymphoedema is a health condition that can significantly impairs quality of life. If conservative medical treatments fail, surgery can be the solution. In this paper we present the results of lymphoedema surgeries performed in our institution.
Patients and methods: All of the genital lymphoedema cases between 2023 and 2024 that underwent surgical intervention at our institute were studied retrospectively. The following data were recorded: age, body mass index, American Society of Anaesthesiologists score, Charlson Comorbidity Index, surgery time, weight of the specimen, length of stay, complications based on the Clavien–Dindo classification.
Results: Seven surgeries were performed during the study period. In 5 cases scrotectomy was performed with the removal of the affected penile skin and reconstruction with split-thickness skin grafting. In 2 cases skin replacement was not performed. The median age was 52 (range: 29–77) years, body mass index was 32.3 (25.0–50.2), the American Society of Anaesthesiologists score was 3 (1–4), the Charlson Comorbidity Index was 3 (0–6), surgical time was 165 (85–250) minutes, the specimen weight was 1851 (574–6000) g, and the length of stay was 5 (3–10) days. Three grade 1 and one grade 2 complication were observed.
Conclusion: The studied patients have been affected by various comorbidities and obesity in addition to genital lymphoedema. In our practice following the removal of pathologically transformed tissues, the remaining healthy skin was closed over the testicles and the penile skin was reconstructed with split-thickness skin graft successfully in all of our patients. After surgery, few and mild complications were observed.
