Urosepsis: Diagnosis and management in light of the 2026 Surviving Sepsis Campaign Guidelines

Summary
Urosepsis is a life-threatening condition associated with organ dysfunction developing secondary to urinary tract infection. Early recognition and timely initiation of empiric antimicrobial and fluid therapy are key determinants of outcome. The aim of this review is to summarize current diagnostic and therapeutic recommendations for urosepsis based on the 2026 Surviving Sepsis Campaign guidelines. It reviews the role of early warning scoring systems in the early recognition of sepsis, as well as current recommendations regarding diagnostics and antibiotic therapy. One of the most important innovations of the guideline is that the timing of antibiotic therapy is determined not uniformly, but according to the probability of sepsis and the severity of the patient’s condition. Current recommendations aim to support early, targeted treatment while reducing the risks associated with unnecessary antibiotic use.
