Experience in endoscopic treatment of large prostates

Summary

Introduction: We will compare the effectiveness of monopolar TURP surgery, one of the operational techniques used for surgical removal of large BPH in our clinic, with the effectiveness of other surgical techniques which can be used according to the European Association of Urologists (EAU) guidelines: bipolar and HoLEP enucleation.

Patients and methods: Monopolar TURP procedures of one urologist for massive BPH were retrospectively evaluated at the Department of Urology, University of Pécs, Hungary between 2019 and 2024 from chart and e-medsol records. Patient age, prostate volume, weight of resected mass, cat­heterization time, hospital stay, rate of postoperative acute, early and late complications were evaluated.

Results: 31 patients were enrolled for the operating sur­geon. Mean age of the patients was 70 years, mean weight of operated prostate was 137 grams, mean duration of catheter was 3.5 days, mean weight of resected prostate tissue transurethral was 57 grams, and postoperative bleeding occurred in 15% of cases, which were managed by balloon compression. No TUR syndrome was seen intraoperatively (glycine solution). There were no procedures that needed to be transfused. Urinary tract infection was observed in 9% of the cases and acute urinary retention in 10% of the cases were observed. No deaths reported as surgical complications. Dysuria and incontinence after surgery were noticed in 30% of patients, in most instances they were treated conservatively, one instance of incontinence more than 3 months was encountered.

Conclusion: Monopolar TURP results for big BPH performed in the Department of Urology, PTE KK are very similar to international literature results on TURP, bipolar enucleation and HoLEP and are not significantly different from results of these procedures.
