The global challenge of antibiotic resistance and the role of immunomodulation in the prevention of rUTI

Summary

Introduction: Antimicrobial resistance (AMR) represents one of the most serious global public health challenges of our time, significantly exacerbated by the overuse of antibiotics. Urinary tract infections (UTIs) contribute substantially to this issue due to their high prevalence and frequent recurrence, which lead to considerable antibiotic consumption. Antibiotic-free preventive therapies, particularly immunomodulation, offer new possibilities for reducing infection rates and limiting the spread of antibiotic resistance.

Methods: Based on a review of the literature and international guidelines, we analyzed the mechanism of action, clinical efficacy, and safety of immunoactive prophylaxis (specifically OM-89) in the prevention of recurrent urinary tract infections.

Results and Discussion: OM-89 is an orally administered immunomodulator based on bacterial lysates that provides long-term protection against urinary tract infections by activating both the innate and adaptive immune systems. Clinical studies have shown that OM-89 therapy has a beneficial effect on reducing infection recurrence and antibiotic use. The preparation is well tolerated, safe, and capable of maintaining long-term efficacy.

Conclusions: Immunoactive prophylaxis, particularly OM-89, offers an effective, safe, and antibiotic-sparing strategy for preventing recurrent urinary tract infections. Its use may significantly contribute to reducing antimicrobial resistance and improving patients’ quality of life.
